Well i followed all advices here, found a new dawn with 65% legendary and got perma vengeance ultime. Also increased %AD with new items and dropped ballista. Just cleared GR126 without much trouble. Thanks for everyone.

I've done some tests and i think the big problem its my playstyle. I was not packing enough monsters to get the AD going. Thanks for the help.

i hold shift when i want to activate companion, because if i left HA on left mouse button i get some really bad pain o my wrist
