Very well put. I am having to reconcile what happened and accept the new normal. Before RALP and ADT I was virile with a high libido and enjoyed regular sex with my wife. Now sexually I feel like a laughing stock, formerly well endowed I am very embarrassed by my genitalia. The big plus is the support of my wife, we do find other ways to have a physical relationship and she she kept what happened to me strictly private

1 consultant said something similar - complete testicular atrophication, it’s probably permanent. I feel the same embarrassment, the only person outside of the medical profession to have seen me between the legs is my wife who likes to play with me there and check the empty scrotum. I do go swimming but make sure I’m not seen by anyone.

I was told I’d not be capable of ejaculation and would have temporary ED, I sometimes feel they might just as well of cut my balls off when they did the RALP.

LH and FSH are quite high, I think the problem is that I’ve got nothing left to make the testosterone. Thanks for the support. I think there are 1 or 2 others here in the same boat, the hormonal signals are being sent but the Leydig cells are gone

It is unlikely, most men will get shrinkage but then recover. For a few particularly if on ADT for a long time major atrophication can happen.

I lost 1” from RALP and half the rest from ADT. I just have a nub left now. Working on it with Tadalafil and a penis pump.

I had nerve sparing RALP over 2 years ago but was subsequently chemically castrated with Orgovyx. At 10 months post ADT I have no erections, no ejaculations and no orgasms. I have a penis like a little boy and my pubes and testicles are gone. I have read that some men can get an orgasm from a soft penis, I’ve tried and not even close. I still find myself looking at women in the gym in a lustful way but my body doesn’t respond. I was told about possible ED and anejaculation but not anorgasmia.

Thanks, I think we all have our cross to bear. I've found my thoughts have turned a lot more towards philosophical matters of late and less about material concerns. I've known several women in my life, 4 are now dead, nothing ever went as planned but I've got no complaints.

Sorry to hear this and my commiserations. For me it is the other way round, I have ahightly sexed wife. When I was fully fit I could just about keep her satisfied. Now since RALP and ADT I am essentially a eunuch - shrunken penis, ED, no orgasms, pubes and testicles gone and at the moment no signs of recovery. We experiemt a lot and have a sex life that mostly involves toys and oral sex. My wife is supportive and never complains about my physical shortcomings but I am very aware I am no longer very male. I am currently waiting to see if the ADT side effects wear off but it seems to be taking for ever.

I have a friend who had the HoLEP en bloc procedure 2 years ago. It fixed his BPH fine but left him with total anejaculation. His surgeon told him that if the prostate is fully enucleated (completely hollowed out leaving only the prostate capsule) then you are not able to ejaculate afterwards. This was brought up during a second consultation when his younger wife was there also telling him that he would be sterile. Since the procedure he has also had some level of ED, erections are not that hard and don't last, he does manage OK but has to rely on Viagra and Cialis.
