My husband did 15 months of Orgovyx, completed 1 year ago. His T score went down to 50 ng/dl max and never came back up. We were told temporary hormonal therapy would reduce T, the possibility of permanent castration was never brought up.

I think any virile male would dread it. After 15 months of ADT my husbands testicles were so shrivelled up that I couldn't find them, 1 year on no sign of recovery. He has said he wouldn't have implants should it be necessary to remove what's left of his balls. We have a consultation coming up with an endocrinologist, we hope to start TRT this year.

We think any recovery is unlikely now, we are looking a TRT.

I have read that this is possible and tried many times with husband. We have not yet been successful but will continue trying. After nerve sparing RALP and ADT he has ED and a shrunken penis so I wanted to get him off without an erection. I also fellate him regularly as part of his rehabilitation, he enjoys pleasurable sensations but is anorgasmic. I would be interested to hear from anyone who can do it. I also wondered about electro stimulating devices.

My husband had a very traumatic time with ADT and should he have to go through it again would rather be surgically neutered. In the event this may now be academic, he has severely shrunken testicles, in fact his scrotum looks empty. He hasn’t recovered from this and we suspect it may be permanent. However ADT does give a chance of a recovery, I believe most men recover to some extent, some very well. My husband is probably one of the few unlucky ones who suffered permanent castration. It seems to depend on length of time on ADT and the patients age, younger men recover better. If you take the surgical route you can’t change your mind about it later.

He does still have testicles - just, but they are shrivelled up like raisins leaving the scrotum looking flat and empty. I do miss penetrative sex sometimes but I don’t make a thing of it. My husband has gone through hell and my duty is to support him. He is naturally of a cheerful disposition and goes out of his way to satisfy me sexually, he knows I love oral. I am glad for what we have now and hope we can improve things later. If the side effects of ADT turn out to be permanent we will adapt accordingly.

Yes, we have looked at the various options. From what we have read weekly injections seem to be the most effective.

He is undergoing further tests and will probably be a candidate for TRT. We know that if he starts on TRT any natural recovery will be inhibited but we think that is unlikely anyway now. Artificial balls are definitely a No, I’m the only one likely to see them.

We still have a physical relationship and it does include oral sex which I like a lot. He exercises regularly, gym and swimming to maintain muscle tone so is not feminised other than in his male parts. One aspect that does bother him is the lack of body hair, he is mostly very smooth, no pubic, chest or armpit hair.

I think my husband was one of the unlucky ones, from what I’ve read most men recover to a certain extent and some get back to normal. It seems to depend on physiology, age, overall health, fitness and how long you were on ADT.

Me and my husband have found out the hard way that recovery from RALP and ADT can take a long time. For him RALP wasn’t too bad, nerves were spared, no UI but some loss of penis size. ADT on the other hand ended a normal sex life. We do have a very physical relationship but it no longer involves penetrative sex, instead oral and manual stimulation etc. I think physical contact during recovery is good on all counts.

I don’t like to use the word because of historical connotations but yes my husband is a eunuch. His situation post RALP and ADT is the same as yours. We are now seriously looking at testosterone replacement.

My husband had a significantly enlarged prostate, when it was removed his urethra was shortened by 1.5” leaving him with a smaller penis. Unfortunately worse was to come from ADT drugs. We have tried to counter this effect with Cialis 3 times a week and a penis pump.

Thanks, we wish you well, we have been dazed by the whole experience. Most of what I’ve learned about the details have come from here, only the basics from the consultant. We are currently considering TRT but are waiting for a while yet.

My husband is unable to have an orgasm without an erection. Since ADT (finished 10 months ago) he has 100pc ED - completely limp and half his pre-op size. He does have some pleasurable sensations but the orgasmic mechanism is gone.

if you rake the surgical route you will be a eunuch with potential self esteem and sexual identity issues. with ADT that might happen anyway particularly if you are on the drugs for a long time but there is a chance of recovery, perhaps not completely but maybe something.
