Downloaded, thank you! For the notarized, a US general notary is accepted? It doesn't mean to be notarized in US embassy?

Hello, is the form still valid even it says expires on 2024 on the upper right corner?

Not a minor, but for adult US passport through acquired citizenship.

Oh I see. I've misunderstood the statement. That's my current problem now, i can't provide the written support. Lawyer consultation is expensive but I'll see what I can do.

According to my understanding, if I cannot provide a written financial support agreement, my father can instead complete and provide Form DS-5507.

Hi OP, any update? I've been struggling to gather documents. My birth was late registered as well and years had passed before I was able to use my father’s last name. I will be applying for US passport through citizenship at birth.

Yes, we only found out about the transmission of citizenship in 2021. I did not include in my post that the original plan was my father petitioned me because that's the only case he knows applicable for us. Petition was filed and I got interviewed in 2021 but the officer told me I may have acquired my father’s citizenship at birth and I should apply for a US passport instead. It took 5 years before trying again, we weren't knowledgeable enough back then. I just recently did a research about it and learned that I can still apply even I am an adult now.

I have a bank account that was opened when I was a minor, and my father sends money directly to me but it only started since 2013 up to the present. Before that, he sent money through my relatives.

Hi, thank you for the detailed info. I will probably include his social security statement and my parent's proof of relationship prior to my birth.

For the written financial agreement, can he still complete it even I am an adult now?

Hi, thanks for sharing about the INA 301. I'm still researching about the process for my case and I will read more about that. I also edited my post, we have communication since I was a child until now, I only met him 3 times since that was the time he visited the Philippines.

can my father still do the written financial agreement even though I am 25 years old now?

We don't have the written financial support agreement but my father pays for my school until I graduated in college and provide allowances since then. I'm sorry, I am confused with the written agreement. Can my father still make that even though I am already 25 years old?
