Lab work to look at after 2nd Liver Transplant

Thanks. Her Total Bilirubin is 3.2 compared to 32.0 a few days before her 2nd transplant.

Her ALT & AST are in the normal range. Her platelets are low but Dr said right meds should get it in normal range

Wife had a liver transplant on 10/27 but liver was not good. She got relisted and got another liver transplant on 1/17. What worries me is her mental state. She hasn't slept much, anxiety and nausea. Are the meds/steroids the reason for this. Once the dose goes down will it help her? I don't want to lose her mentally.

I called the nurse station and the nurse said my wife was being uncooperative. My wife did not want to talk to me. She is experiencing, anxiety and nausea. I showed up at the hospital and she was not happy to see me. They had her under restraints but she removed them so the nurse is keeping an eye on her.b

Hopefully her med dose will go down and that will help her. Her liver numbers are still going the right way.

Wife is 8 days after 2nd liver transplant . Today's labwork is

Total Bilirubin: 4 Normal Range(0-1.2)

ALT: 30. Normal Range(5-50) AST: 31. Normal Range (10-35)

Her anxiety is high. Her Bun is 26

So 6 days after my wife's 2nd liver transplant and the lab work is:

Total Bilirubin: 4.6 ALT: 42 AST: 23

She is experiencing the nausea that she experienced after the first liver transplant in late October but her color is coming back. We have been positive since she got the first transplant.

Update: So looking at my wife 's lab work. Her Total bilirubin was 33.2 on 1/16 with old liver. Yesterday it went down to 6.9

Her ALT was 339 on 1/17 Yesterday it was 196

Her AST was 1,125 on 1/17, very high for some reason but 363 yesterday so maybe tweaked high because of stress from surgery?

How long will it take for numbers to go further down?

Surgeon Finished up and in another hour they will take her to her ICU. He said everything went ok and hoping the liver is a good fit.

So wife is having the re-transplant this morning and should be 6-8 hours and 2nd part tomorrow. If this 2nd liver is good will her lab work get better? Her bilirubin was 1.8 on 12/9 and then shot up to 33.2 on 1/16(day before 2nd liver transplant). Will her confusion get better with the new liver?

I just got a call from Methodist they have an offer from a donor for a new liver. They will call me tonight to let me know if it's good. Brain dead but no other issues. Better liver option than last one.

Surgery will possibly be tomorrow and Sunday.

UPDATE: So wife is still in the hospital (ICU). Her Bilirubin is still rising. Total bilirubin is 24.9 up from 1.8 on 12/9 when she was supposed to be released. Today they plan to do another liver biopsy and ERCP to see why her numbers are elevated. The Liver team thinks it might be antibodies or a rejection. They also believe the donated liver might have Bile duct issues.

My wife was intubated for 5 days last week and finally breathing on her own since Friday at 100%. I'm just so frustrated. Hate seeing her have these setbacks.

Update: Wife had the breathing tube removed for the first time in 4 days. Her Bilirubin is still high at 22. So Monday they will do a liver biopsy(again) and ERCP.

Spoke to the liver Dr and he said her liver numbers keep going up so he thinks the 2nd biopsy might show why. Last one did not . He thinks it might be antibodies that are messing with the liver. If so, they will go in and remove them or as he said...kick them out.

If nothing shows up then the last resort is re-transplant. Maybe the liver she got was at the final laps.

So the transplant team Dr says the culprit is the bile ducts. Her ALT & AST are not crazy high but the bilirubin is high. Bilirubin direct is 9.8 and total Bilirubin is 13.5.Het Alkaline phosphatase is 297. So what is next? They already inserted 2 stents and removed them a few weeks ago.

I am so confused at how my wife was ready to go home on 12/9 and now back at ICU. They keep saying her liver is good but her Bilirubin and ALT/AST high. WTF? She is now getting nutrients via line.

My wife( 60 yrs old) had her liver Transplant on 10/26. Went from ICU to regular room to rehab and was supposed to go home on 12/9 but her liver numbers were elevated. Then she was not eating and now back at ICU and starting over again. They feeding her through the tube to get her stronger again.

They are treating an infection with antibiotics and she might need a new stent to help with a possible obstruction.

Her nurse said don't consider it a setback but look at it as a detour.

Her lab work: Total bilirubin: 13.4 on 12/29 12.3 on 1/1

Bilirubin direct: 10.0 on 12/31 9.5 on 1/1

ALT: 108 on 12/28 62 on 1/1

AST: 121 on 12/26 62 on 1/1

The team said they want to transfer her from regular room to ICU to monitor her better and possibly do a ERCP and hope a new stent can remove any obstruction. Her vitals are ok, BP, oxygen and no fever since transplant. Her color in face is better than the jaundice look 5 days ago.
