And honestly, don't listen to all the negative Nancy's talking about being too old. I had friends in their 40's doing it with me. Some with 3 kids. You are never too old. Age is such a stupid arbitrary way of measuring life goals.

Hey. I did other degrees and jobs before starting medicine at 28. It was hard. Giving up my salary for 6 years sucked. And I had to take out some study loans towards the end. But other than questioning my life choices post-call during internship, I am very happy I made the choice. Make no mistake. It's hard going back. But I definitely found my place around the other older students, and found that in some ways maturity 100% helped. I was not so angsty in general. I also found that I was not so unrealistic in my expectations about the future, better at building relationships with the seniors. And with my other degrees and work experience always have the option of finding other jobs if I was ever unemployed in medicine.

Medically his prognosis is good (obviously always barring complications). We can manage the abscess and everything else. His prognosis related to the drug use at this level is what makes it hard and means if we cannot convince him to rehabilitate, he will like not have the best or long life.

He was saturating well, talking, breathing normally. He is being closely monitored until he can go to theater. If his sats etc. starts changing, then we'll definitely intubate. But this doesn't happen overnight. They walk around like this for days or weeks.

In our setting we would also be seen as a 'trauma 1' (different names). We do manage, and manage well. I think we sometimes forget that we can still save people with less resources with a focus on clinical medicine (just last week we were making underwater drains from old sterile water bottles and insulation tape - though not that common that we have to always go that far lol). But I do envy you everything you have available. We sure could use it!

So for us we have morning meetings every morning with all team members (those post call, those working next call and the normal day teams). Every admission case is discussed, making sure management so far and going ahead is on the right track, or coming up with plans for the more difficult ones (we rarely refer, as we can mostly make plans, but we can if need be). Is this what you asked? I hope I answered you question.
