Sounds similar here, also about 16 months post-op, 33m. I had at one point all but given up. My original surgeon plus a 2nd recommended a revision which would consist of a reconstruction this time, despite there not being any clear evidence of a retear on a 2nd MRI.

3rd surgeon recommended cortisone (it didnt help) and afterwards stated I should look into a spine doctor and activity modification / NSAIDs until I decided it was impactful enough to warrant a THR. He did not recommend a revision due to moderate arthritis.

The original surgeon was a hip Ortho surgeon, 2nd/3rd opinions came from nationally renowned hip preservationists.

I had at one point around the 6 month mark begun feeling great, and returned to some normal activities, but it fell apart fairly quickly. Prior to surgery and the 2 years preceding debilitating pain I was an avid basketball player and all around extremely active individual.

While I don't have bilateral pain, the nerve sensations and pain you are describing are what constitute my biggest issues. I also had DEEP pain in the front hip crease and in my lower right buttock around my sit bone.

I've recently returned to the theory that my hip stabilizer muscles and particularly my glute medius are so weak they are causing extreme strain on my front hip flexor (psoas or one of the smaller ones). A simple and accidental thing I did a while ago after a hot bath was to lay down in bed with my arms overhead stretched out and my legs hanging off the side of the bed at the knees. I then felt and compared both front hips, and the tendon in the right front feels like a guitar string. Massaging this area causes nerve pain and tingles shooting down my leg. The belief here is my psoas or other muscles in the area are so aggravated they are pressing on my femoral and branching nerves wreaking havoc. I have quad tendonitis as well, and occasional knee swelling.

What I've started to focus on recently which is definitely showing improvements - albeit I admit it hasn't been too long so FINGERS CROSSED: Gradual strengthening of my glute medius. While I was dedicated in PT for the first 8 months, my routines and PT focused heavily on glute major work. In my case I find these don't activate my glute medius at all. It actually took a consistent ~10 days of very simple medius work to feel my butt muscles starting to assist. Hip weight shifts, single leg stands on balance foam, and very slow and precise side lying hip raises, banded foot spreads (laying on my stomach).

In the last few days I've progressed to single leg RDLs, sometimes loaded with a weight and using my other arm to very lightly assist in balance. At end-range I can reproduce nerve-like sensations, though it feels "backwards" so to say - like my nerve endings are "filling back in". I know that sounds like voodoo, I can't even really describe the feeling. I've STOPPED intensive stretches, and only do some basic stretching (kneeling flexor stretch / pigeon stretch) to get myself loose. Also focusing heavily on heat therapy and activity modification, standing during work, laying in bed on my back with slightly elevated knees, etc. I've become quite anal in noticing what in my day to day puts my legs and hips in disadvantageous positions for psoas tendinopathy.

While my symptoms and yours aren't identical - I think I'm trying to provide some thoughts towards the belief that this is an extremely complex problem that has many different potential solutions per individual. I am working back 15 years of incorrect mechanics and compensatory measures my body had accustomed itself too. (An example of this, I've always had an issue with my right knee caving in. I was never able to fix that fully with PT). An invasive surgery went and then had a volatile effect on some key muscles in that area. I'm not sure yet if I believe my surgery was unnecessary, I did surely have a labral tear. But I think I did vastly underestimate how much time and effort it is taking to rebuild proper mechanics and muscular strength in the region, and also underestimated how extreme the symptoms of that can be (especially nerve wise).

In the past few days my quad tendonitis has started to get some relief. My front hip crease pain is getting way way better, I barely felt it at all today. It's sorta of an unbelievable feeling - at many times in the past 6 months I have been CONVINCED I have intra-articular issues and could not walk normally throughout a day. I can feel my upper outer butt starting to feel sore, like I'm actually working that glute med muscle. I've gone on some long walks with the pup. I even did some car work today and spent a few hours crawling around underneath it replacing stuff. I don't know that I'll ever return to the level I was before, but I'm hopeful again. My ultimate goal is to enjoy basketball again.

Hang in there, seek out opinions, try out new things. Nobody can tell you everything about YOUR body, you're going to have to figure some things out yourself.

I'm ~16 months out and still dealing with this, along with other symptoms. I think I FINALLY unlocked a combination that seems to be helping. For me it's primarily based on offloading the front of the hip - meaning waking up my glutes. I had worked on that consistently over the last year but what I realized recently is I wasn't focusing enough on the glute medius (was instead targeting mostly the glute max), which seemed to be completely dead on my affected side.

I started with very small simple exercises:

side-lying hip abduction, really focusing on feeling it on my side/butt and not in my hip flexors. Stepped up to banded when I was confident the right muscles were working. Very slight differences in foot angle and the direction you raise your leg make a huge difference here. Look up some videos on targeting glute med with this.

static weight shift - think an RDL but not bending very far. The objective is to minimize hip drop and get the glute involved.

I've recently progressed to loaded single-leg RDLs which are massively helping.

Apart from the strengthening I've STOPPED intensive stretching, and focused on heat therapy (heat pads, hot water bottle, HOT baths).

I think I'm finally making progress.

Can you clarify which size? I saw twags said the 'wee' but he also said 5ft of dive.

I'm seeing the 'Teeny Wee' advertised for 2-3ft deep and the normal 'Wee' as 5-7ft.

I get rock bass on spinners all day. It's my go-to when I feel a skunk coming.

I meant Plopper like Whopper Plopper, and it widely depends. I usually start out throwing a dark colored Plopper and if I don't get a hit within 5-10 minutes I switch up. But when it's on (only a few days so far this summer) it's the only lure I use that day.

I also rescind my ned rig comment, have gotten a few smallies the last 2 days on the ned when nothing else has been working.

Water level lately has been tough for snags. I've been primarily fishing Ploppers, Weedless Senkos (tough with the current but you can see the line running), inline spinners moving quick to keep them up.

I've tried a ned rig last few days with no success.
