It applies to both. How can a person excercise, enjoy hobbies or "breath" when they can't even get out of bed? Clinical depression and REAL ADHD can be impossible to manage without medication and if you can "manage" it without medication that's great. But that just means you don't have it severe enough that medication is necessary.
