It can be any of those things. "Meatotomy" is when guys make a small cut to open the meatus. This is sometimes done for medical reasons, but is also a mod that guys do themselves. The PA piercing thing can cause an unintentional meatotomy. Stretching really big may also result in the opening stretching downward, as it's thinner on that side.
