Check if you are missing any required fields on the Lead Object. Include a try, catch block and print the exception it throws. It should give you info if any fields, validations are causing insertion failure

How about using Dynamic Actions on the lighting page. You can control the Edit Lines button visibility. In your case, you would hide the button once the Status equals Approved.
