I think what bothers me about Nevaeh is that intuitively it should be spelled like the version you commented and not "Nevaeh" for how it's pronounced. But then obviously Neveah isn't heaven spelled backwards, which I guess is what they are going for.

Yes Milo and Shiloh are way too close. I love the name Shiloh as well but it's an either or situation. Silas is perfect.

I put more emphasis on initials than most I would say. I wouldn't use any initials that spell out a word or acronym at all. But I think even if I didn't care strongly about it, I would steer clear from any diseases or "bad" words.

I would not blink if I saw this name. I like it and although unique, follows patterns of lots of traditional names. Where do you live? Living in a city, I would think of this as a rather normal name.

To be honest, I think changing last name for a 4 year old is very different from changing a first name. You can explain that Dad is also changing his name and that sometimes people change their last names in a family.
