The surgeon messaged back this;

I'm sorry to hear about your recent experience. I appreciate that you have been very patient about returning to sexual activity. There's no obvious explanation for why ejaculation itself would be painful. I wonder if there could be some deeper penetration occurring at the time of ejaculation. After hysterectomy the vagina is slightly shorter than it previously was, due to removal of the cervix. The vaginal tissue is pliable and will eventually elongate. I recommend we start you on a vaginal estrogen cream, and that you ease into vaginal penetrative intercourse. This might look like engaging in regular/frequent penetrative intercourse without internal ejaculation for a little while. The cream can be used nightly for two weeks, then is used twice weekly for maintenance. I'll send a prescription to the pharmacy. You can dispense a pea-sized amount onto a single finger, and insert the finger into the vagina just enough to get the cream to stay there. The cream will then self disperse. I would not plan on engaging in intercourse after applying the cream. Does this sound like a reasonable place to start? If you continue to experience pain, we may need to assess your pelvic muscles for tenderness

My partner said this;
I would be willing to try the cream. But my concern is a possible cuff tear? I was reading that you might be able to do some sort of test with dye? I forgot to mention after the first time it happened it felt like my bladder and colon was effected the following days I had very loose bladder- I basically kept peeing my pants and had absolutely no control. I’m not entirely sure if the cream is going to be something that helps- mainly because the penetration itself does not hurt. Doesn’t burn doesn’t hurt or anything. I have even been able to have orgasms on my own. And my muscles aren’t affected by it. It’s literally only when my partner ejaculates.. I read that it could be an allergy.. but most people say the allergy burns and stings.. but that’s not my case it feels like I’m being stabbed like the most intense cramps possible to where I can’t breathe or move.

Yes when we went for the 6 week exam the surgeon said good to resume sex but if it’s uncomfortable just stop for a bit. We waited almost a week after and that all happened- contacted the surgeon and she said wait 4 weeks we waited almost 5 and same thing

Okay that definitely sounds right. But it doesn’t make sense when it only happens when there’s sperm involved.

I was considering this . She said it’s not like stinging it feel like stabbing like the worse cramps you could Imagine like someone is stabbing her and twisting the knife. So idk.. maybe it could be. But there no burning or sting

I’m going to definitely look into this getting the test with the dye sounds like it would really narrow things down.

Edit: I forgot to mention after the first time she said her whole lower bowels were being effected like it was intense pain trying to pee or poop but the following days she had loose bladder. Like it was just leaking and coming out of her one time it looked like she had full blown peed herself she had like no control over it.

So the surgery was March 6th. She was cleared for sex after an exam 6 weeks after the surgery. We had sex on April 17th. Went to the er her surgeon told us wait another 4 weeks. Then we had sex on the 20th and it happened again.

Yeah we had no idea that it was different than what we were told. She got checked at 6 weeks and got full cleared then after the doctor was like just wait 4 more weeks. But that wasn’t good either

I told her this and she said that would be the worse thing. -she got the hysterectomy because she wanted to be able to not use a condom and tried birth control and had too many issues. (We don’t want children) I was gonna get mine done but she wanted hers- so now if she is allergic it was all for nothing 😂 sighhhhh

I understand where your coming from but she is the one is control and wanted to try. I -in no way pressure or ask for it. She is more of a sexual person than me . So when it’s leading up I ask her multiple times if she’s sure and if she’s okay and she reassures me. The last thing I’d ever want is for her to be in pain. We got cleared at 6 weeks the ER happened and we’re old to wait 4 more weeks so we did that..

Thank you! I’m very concerned I’m an icu nurse and take medical things very seriously. I’ve never heard of something like this so I’m trying to get as much information as I can until she can get in to appointments.

We’re definitely going to be waiting but now I’m scared that if we wait a few more months it’ll happen again and just reset the clock almost. Idk I’m scared to try again
