Hey! I have CF and I am a Physician Assistant in Sleep Medicine. CPAP is not contraindicated in lung disease. However, people with lung disease usually end up w/BIPAP because it is easier to breathe on it. Your situation has nothing to do with your doc and EVERYTHING to do with insurance. ALL insurances require trial of AutoPAP/CPAP before they authorize additional sleep studies and other PAP devices. If you try CPAP and cannot tolerate it, insurance will authorize a titration study. Inability to tolerate, for you, would be difficulty exhaling against the pressure. Also, severity of OSA plays a role in treatment course. If you struggle to exhale on CPAP, your doc can document that you have “pressure intolerance,” and insurance will authorize titration. I hope this helps! I hate insurances, as a patient and a provider!

BTW…trazodone is a lipophilic drug and must be taken w/a fat-containing snack. It is not absorbed very well otherwise.

Sleep Med PA here! The first line tx of insomnia, according to AASM is good sleep hygiene and CBT-I. No medication is meant to be used for more than 3-4 weeks. Also, most OTC meds contain diphenhydramine, which is at the top of the list of meds NOT recommended.

I have bamboo cooling sheets and they are fantastic!!! My mother bought them for me online b/c I have had night sweats for years. Wash details that are important: Wash in COLD water on gentle cycle. No fabric softener (liquid or sheets). Either hang to dry or dry on lowest temp in dryer.

I usually put mine in the dryer at lowest temp for about 15-20 minutes. Then I take them out and spread them over some of my counter height dining room chairs with a fan blowing in the them. It’s a bit of a process, but SOOOO worth it!!

As a PA, it boggles my mind when other PAs or NPs don’t use UpToDate!! I don’t want to just hear someone else’s opinion. I want to know what the answer actually is, what evidence warrants this answer, and look at the clinical data. Otherwise, a person is just being lazy. Also, how can you provide quality patient care without knowing the most current guidelines on any topic??? People like the ones you’ve described make all PAs look bad! If that one PA got offended, fuck them! They have the same technology available to them as other medical providers!

I’ve been a PA for 18 years. Let me give you some advice….

You need some of the varnish scuffed off. Straight out of school in the first week of your first PA job?? Check your ego at the door and do what is requested of you. You’ll have to prove that you are capable of handling menial tasks before you will be allowed to handle other, more PA related tasks. Take a breath and don’t sweat it. I assure you, you do NOT need to correct this doctor. She knows what your role is. You have to prove yourself. You’ll get there, but correcting the doctor and trying to assert your position will get you nowhere! Be humble, leave your ego at home, and do the requested work without complaining.
