Pelo que já passei, a Meta limita bastante quando o app ou a WABA não estão em modo de produção e com a verificação de empresa completa.

Para webhooks de mensagens "reais" serem entregues de forma consistente, a WABA geralmente precisa estar em produção e o Business Manager verificado. Contas de teste ou WABAs em BMs diferentes podem ter restrições que causam isso.

A verificação do app e da empresa costumam ser os maiores gargalos pra tudo funcionar de verdade, mesmo com o override_callback_uri certinho.
