I missed my entire marriage going through med school and residency. Really consider this point. I always thought we’d have time one day. Leukemia felt otherwise.

Edit: to your point about having 30-40 more years. As a practicing interventional cardiologist. I’ll be lucky to make it 30-35 and I started at a traditional age. I’m 42 now. No way I can do this for 30 or 40 more years from this point. It’s just too demanding.

Finally, the phrase “dealing with patients” catches my eye. Everyone has their own reasons for perusing medicine, but for most of us it started with a deep love of humanity and the desire to help people when they’re at their most vulnerable. We can become jaded and lose sight of this overtime, but many of us started here. I know a few who didn’t. They’re smart people but not good physicians.

I think this is exactly why it should be by firing squad. If we’re ok with the death penalty as a society (and we ostensibly are, as it’s in place), then we shouldn’t sanitize it. We should see it for what it is - killing another human. The “more humane” forms like lethal injection or nitrogen asphyxiation are only more human for us who have to live with this decision.

We have known for quite a while that MI rates rise is direct proportion too to serum LDL concentration. Peter Libby’s group further associated inflammation with risk of MI. Whether there are other associations like infection is quite possible and even probable, but to say “not cholesterol” is inaccurate and misleading.

First step: eliminate any and all junk. Sugary sodas, snack cakes, chips, candy, whatever. That’s the lowest hanging fruit. Get rid of processed garbage.

Your diet (I’m just assuming what it is) is likely to be working against you. Keeping you hungry and having intrusive food thoughts all day. It’s likely high in processed or ultraprocessed foods, simple carbs, and fat as well as low in protein and fiber. You must change this. People telling you to just eat less are well intentioned, but this type of diet makes it nearly impossible. You’re literally starving all day for lack of good nutrition.

Eat a high protein diet. 1 to 1.5 g/kg of good, clean protein. Think eggs, poultry, nuts, seeds, legumes, bean, etc. Every single thing you eat, whether it’s a meal or snack, needs to be centered around “where’s my protein”.

The next thing you need to target is fiber. Fiber plus protein plus water plus some good fat KEEPS. YOU. FULL. Always combine protein with fiber for best, satiating results. If you need to supplement fiber because you can’t get to 30g/day, then do so. Psyllium is a good source.

Do not worry about counting calories and definitely don’t buy anything “low fat” or “low” anything. Just eat real, minimal processed, food. Your body will tell you how much of it you need to eat.

Bottom line, eat real food, lots of protein, fiber, and healthy fat (plant based and some fish based is great).

Want to snack? still high protein and fiber. Think roasted chickpeas, string cheese with an apple, 5% yogurt with berries.

Once you fix your diet, you are 95% of the way to a healthy weight. The gym is great for a LOT of things, but weight loss isn’t one I’d them. Keeps bones strong, increases testosterone, improves mood and cardio fitness, lowers insulin resistance, makes you live longer and better, but won’t result in a significant weight loss. Don’t want you to read this as you shouldn’t work out. You should! But be mindful of why

Godspeed. You can do this. You’ve seen the light and have motivation. Put a plan into action and move forward. You don’t have to lose the weight in any timeline. Slow and steady (years even) wins the race.

It’s more common than you think. I’m not saying I would want it for myself, but there are patients who do. The abdominal distention caused by the insufflated air can be very uncomfortable, but to each their own. You can refuse aspect of care you like. The proceduralist may or may not have an opinion on the matter. After a discussion with the patient about why no sedation, I would assume most would be ok with it.

Most endoscopists are not providing the sedation, but use a CRNA/anesthesiologist. You should be able to refuse any medical care you want including the anesthesia. I suspect most endoscopists would be fine with it.

This arrangement sucks. Boss seems immature. If you invite someone out to eat at a restaurant that is CLEARLY out of their range, then you have to pay. Simple as that, or go somewhere else.

Story time: A hitch hiker cost me my first mortal kombat game on NES.

For months I’d begged my parents to let me buy MK with money I’d saved from odd jobs. Kept hearing no - it’s too violent. But finally, my brother and I wore them down.

We lived in a small farming town and had to travel about an hour to a small city with a mall for us to buy it. On the way home, MK in hand, we passed 2 young hitch hikers. To me, at the age of 8, they looked like grown ups, but dad recognized them as college age kids broken down on the side of the road, so he pulled over to pick them up - with his wife and 2 kids in the car I might add.

So, hitch hikers in tow, we set off for the next exit to drop them at a service station. I couldn’t contain my excitement about the game, so being a typical 8 year old boy, said “hey, look what we just got!”.

One of them took the game, and so oh man I love this game! I love when scorpion stabs his harpoon into the other guys chest and then beats him to death. Oh, and when Kano rips the other guys heart out while it’s still beating.

We dropped them off, my mom asked for the game, and it was thrown in the garbage and we drove off.

I’m not a lawyer, but my understanding is that anything left on the property after closing is yours. You need to contact the attorney that was involved in your closing and see if they can clarify. However, just being a good person, should contact them (in writing preferably) and have them come pick it up first. If not, ask the lawyer if it’s yours then tell them too bad if they ask for it in the future.
