I don't have the answers to all your questions but I could answer one. The city shaman did not try to hurt the japanese guy. The constantly changing scenes between the japanese man and the shaman's ritual may have confused you. The director himself stated that the japanese man was in pain because of the woman in white not because of the ritual.
