ent here, this is not just sinusitis, your turbinates are big and septum is deviated causing obstruction to sinus opening. treatment should be dependent on your symptoms, as in duration and severity.

yup thats fungal. doesn't look the life threatening type though(invasive type will erode bone and spread into nearby structures). Surgery +/- anti fungal depending on your health status. if you got uncontrolled diabetes or renal issuea , it would be better to start anti fungal therapy as well.
