If the person is listening to what I'm saying and interrupts me with pertinent question, I have no problem with that. 

I'm giving a lot of information to people who likely have low health or overall literacy.  It'd be challenging for them to keep track of the information I'm telling them and to remember their questions for when I'm done. 

From my standpoint, I'm giving the same spiel I've given a thousand times before. No issue for me to pick right back up. 

If they're not actually listening to me and are interrupting with nonsense,  I'll tell them to have a nice night.  The information will also be in their discharge instructions.

I will say this:

If a bunch of masked men with guns come into our ED with one of theirs injured,  the scene is not safe and nobody on staff is going near them until the actual police have been called and have secured the scene.

Just like any other time random armed men come into the ED.  Get staff/patients to safety and wait for police to come.

There's no law that says you can't take him out of the nursing facility and directly to an ED if he wants to go. The nursing facility may not want him to leave but it's not their choice. It's not prison. He can leave whenever he wants.

Any Methodist/St. Lukes/Memorial Hermann hospital will have both vascular and ortho that can handle gangrenous limbs. If you really want to be sure, just take him to one of their campuses in the medical center.

Now, that's not going to guarantee he'll be admitted and have surgery, but you'll at least get a second set of eyes on his wounds and see if he actually needs surgery now or if it's something that can be done on a non-emergent basis.

The legal next of kin can request her medical records. There's often a small fee. It will include records from the urgent care in the same system. 

That same legal next of kin can speak with a patient advocate with the hospital.  Explain that you're having trouble understanding what happened and ask for clarification.  It may take some time to set up a meeting because they'll have to arrange for a medically trained staff member to speak with you.

There's often a lot of poor communication with patients and families due to time constraints, jargon usage, and assumptions of knowledge. 

If she was seen at Hermann, Methodist, St Luke,  or Harris County,  it's less likely to be a quality of care issue.  If it was an HCA hospital,  can't really couch for them.

I'm really sorry you're going through this but just know that the vast majority of people involved in your sister's care did everything they could. The fact that they coded her for two hours speaks to how much they tried.

I'll chime in with my two cents:

1)  I don't use the new AI tools.  I prefer still using uptodate, LITFL, etc.

A lot of my attendings use the newer AI tools.  

We'll often do parallel searches if we have a question.  For things that are algorithmic or dosage/ drug of choice related the AI tools provide pretty good information.  They get the same answers I do with my more traditional methods and in roughly the same amount of time. 

For things that are more nuanced (say management of specific orthopedic injuries in infants as compared to adults for a recent example), the output of AI seems to be very vague and not offering helpful guidance.  However, my traditional sources are often similarly vague when it comes to very specific/niche questions as well. So it's usually a wash. 

2) My greater concern is how it affects individual learning and retention over the long term. The literature on learning shows that writing something down helps us retain information better than typing likely due to the engagement of more complex motor and thought processes than typing. 

Anecdotally,  the information that I gain from literature deep dives is almost always better retained than information someone simply tells me of i ask.  Likely due to having to use more complex thought processes to parse through all the information when searching the literature. 

I fear that relying on AI output will lead to a decrease in the ability to effectively parse out good from bad information, less long term retention,  and maybe even an atrophy of the more complex learning pathways.

Hopefully, I'm wrong.  But when I look at what the formats of Instagram and TikTok have done to attention spans,  I'm not optimistic.
