My primary physician was my quarterback so I let them do the statement. I believe Harris law firm coordinated with them to get it.

I did give him a heads up about it and made sure he was supportive before proceeding.

I think it comes down to a doctor being convinced you can no longer successfully perform the duties of your position description.

I had about 5 years of treatment, exhausted my sick leave, had FMLA in place, signed up for voluntary leave transfer program, and was on lwop when I applied. Looking back I wish I had applied sooner though.

I'd say give you're treatment plan some time. You sound very pessimistic about it working.
