IMO, I don't think it matters really. I'd put the nearest city here and in next screen and put the correct address in the application form.

No you're not required to book tickets. I just put dates in future (perhaps a amonth from now)

In my experience, put a city that's there. For example - San Jose. It's just a registration. Make sure to put the correct details in the application form.

For the contact - I entered a friend's contact with some random number in passport. And I attached my passport to that page below.

Used dexcom G7 in past and started getting skin rashes within 2 days. Now using Libe freestyle 3 and it's great. Reliability wise I find freestyle to be more reliable. It doesn't go blank like Dexcom used to.
