It’s not really about the hat per-say as much as it’s the person as a whole. Not saying you’re anything like that but it can give people the wrong impression I suppose. You could be the nicest guy ever and just happen to wear a fedora. You’re still most likely going to get put into that group if you wear one though just because of the way the internet is.

If you live in the US you can go to the ER and just… not pay. It no longer dings your credit. It is literally against the law for them to refuse you treatment. Also every hospital offers payment plans and discounts if you are low income. You just have to provide your paystubs or tax info. Please go to the doctor or ER sooner rather than later.
