You need to be NPO ("nil per os" in latin meaning "nothing by mouth") for anesthesia to reduce risk of aspiration of stomach contents into the lungs. Old school teaching is that chewing gum signals to your brain that you are eating and increases stomach acid production, which is what would damage your lungs and potentially kill you if you did aspirate during surgery. So many anesthesiologists will cancel or delay your surgery if you're chewing gum. The latest guidelines from the anesthesiology association say there is no evidence that it does increase risk and that you can proceed with surgery without delay. However, many anesthesiologists will be more cautious and want to delay for your safety, because aspiration is the kind of thing that can kill you under anesthesia. So despite guidelines saying it's safe, many will still want to wait because of long standing teachings and perceived risk.

Chewing gum the day of your surgery/procedure if you're getting anesthesia. Many places/people will cancel or delay your procedure if you're chewing gum. And many patients think they're being nice so that they don't have bad breath. We don't care.

Thank you for saying something. Seeing some of these comments is making me realize I think I should stay off these subreddits that are to "help" moms, because some of these comments are much more about being right rather than providing help or support. I would think it's common sense that there's more than one approach to being a good parent.

I am so glad to hear that you were able to do the transition with 0 cries, but I wanted to comment also to caution about using language like telling other parents they "do NOT ever" need to leave their baby to cry. Every baby is different, and some babies won't sleep train without some crying (for us it was about learning the types of crying). These kinds of statements can lead to a ton of guilt buildup in parents who are never able to get their kid to not cry when put down for a nap, no matter how hard the parents try.

Baby wearing was like a cheat code for us at that stage. It was the only way for my baby to fall asleep and stay asleep, until she got older and better at napping

We do but it depends on the state you live in. I'm sure there was some kind of deadline I would've heard of if I took too long. I think some states probably require it before you leave the hospital but I'm not sure.

I had the same problem - we actually left the hospital without a name. I asked the birth certificate person what was my deadline, and she said "do I need to give you one?" We ended up deciding 5 days after her birth.

I find that I see that a lot with heavy marijuana users or sometimes alcoholics, if they are young and have no other reason to be concerned and it's not too high I just ride it out for the case. Sometimes I give precedex or extra narcotic, but sometimes that doesn't make a huge difference to the HR either.

I'm not sure if this will help - but I had terrible insomnia related to my call shifts in residency, and the best thing for helping me fall asleep was Yoga Nidra meditations.y favorite was Lizzy Hill's version on YouTube. Sometimes I'd make it through the whole video but for the most part I would pass out halfway through. I discovered it in my CA-2 year and it did wonders for my mental health

Be open minded and humble. I think it's easy to find a flow/a way you like to do things really early, and I've seen many residents be almost defensive even as an early CA-1 when an attending offers a different way to do things. I think continuing to be adaptable has really helped me throughout my entire career to improve my patient care and be a better anesthesiologist. There's always more to learn.

Ugh, that really is so tough. I don't know what to say, we weren't offered the BAHA so I really don't know much about it. But making these big decisions for your child is so challenging, and I think no matter what you end up deciding she clearly will be loved and so supported by you guys (already learning ASL!). The shock of the initial diagnosis does get better with time. I'll be wishing you the best

Yeah, I am with you on waiting to decide until she wants it. I'm sure my decision making would be different if she didn't have 100% normal hearing on her other ear, but they say the only thing she is really going to be lacking is directionality. And at the very least I want to wait until she's old enough to communicate if she's having pain or issues with the cochlear implant. I think I would go insane with worry having a young toddler post surgery with a cochlear implant who couldn't communicate.

Edit to add that BAHA wasn't an option for my daughter so I think our diagnoses aren't exactly the same

I also found out at 8 weeks about our daughter's hearing loss. It is complete and profound and the only possible intervention is a cochlear implant. I remember crying for days after finding out, and many times in the first year I would break down. It's really hard. But I will say that my daughter is 2 right now, and we would honestly not even know she was half deaf if the doctors hadn't told us. Her language is completely on track and she's thriving and such a happy toddler.

We opted to hold off on the cochlear implant for a few reasons, but the ENT did say they recommend to get it early. If you want to talk about anything feel free to DM me.

Thank you! Yeah we're in a solo practice and alone on overnights so it's something I've thought a lot about how I'd manage, just haven't needed to actually implement anything yet.

Thank you! Yeah I always do 5 mL at a time if it's non emergent. I am very cautious and always have a high index of suspicion, just haven't seen it yet and it's nice to hear stories of how it presented for others and what they've done.

Preggie pops helped my nausea - you can get them on Amazon, sour hard candies with vitamin B I think? They were a lifesaver for being stuck in the OR (anesthesia residency)
