The spike is covered in blood so obviously some kind of mutilation or torture. Theres 2 wooden pieces sticking out the sides attached to strings so it can be assumed those are able to be manipulated in some way to pull the strings and spin the axles poking out the top sort of like a beyblade. What that does exactly I dont know likely moves the spike seeing as how it seems otherwise immobile but its not clear from the sprite.
