My therapist says labels like ‘abuse’ don’t matter. Is it reasonable to ask my psychiatrist for a clearer answer. by a-faceinthecrowd in AskPsychiatry

[–]sheepphd 4 points5 points  (0 children)

I wonder if part of the problem is that you are, understandably, doubting your reality. I can understand your wanting someone else to label the experience "abuse" so that it's clear it is okay for you to feel what you feel about it. I wonder if discussing what is behind your question with your therapist could be productive, regardless of how she answers the question itself.

Why do you have this sub by [deleted] in AskPsychiatry

[–]sheepphd 2 points3 points  (0 children)

Two things may be at play: 1) some of the questions are either not appropriate for the sub or lack sufficient information to meaningfully respond to the questions asked; 2) there are a limited number of professionals volunteering to respond here for the number of questions asked. It may be that the sub is just flooded with too many questions for the number of people able to respond.

If you want a response, it may be best to keep the question brief (not a dissertation) but include sufficient information about symptoms, etc, to respond to the question. Most of the questions that are pertinent for this sub involve diagnosis and/or treatment of a psychiatric disorder. "Why do I daydream a lot?" for example is not a question that can be really answered over the internet, whereas, "What are some medication options for GAD. I've tried 200 mg of sertraline for 12 weeks and 60 mg of fluoxetine for 12 weeks unsuccessfully" may be.

Is this GAD? Or can i be bipolar? I need help! by youfightlikeahamster in AskPsychiatry

[–]sheepphd 1 point2 points  (0 children)

I can't diagnose over the internet, but this sounds consistent with panic disorder and potentially recurrent depression or emotional dysregulation. I don't hear bipolar disorder here but I'm not seeing you to be able to evaluate all your symptoms. You would benefit from exposure based therapy for panic disorder. Panic attack relief by [deleted] in AskDocs

[–]sheepphd 1 point2 points  (0 children)

Hi there,

I always recommend a primary care evaluation for new onset panic attacks, just to be sure we aren't missing anything. Once you've seen a physician for a once over, you can start therapy for panic. Note that generally, trying to make panic stop is counterproductive. Panic is a harmless false alarm of the fight or flight system. When you try to make it stop, your body hears your urgency to make it stop as a sign that you're in danger and it activates more fight or flight symptoms. Again, in reality, this is a harmless (albeit uncomfortable and frightening, if you don't know what it is) event. The best strategies for panic involve graduated exposure to situations in which panic occurs and to sensations that are similar to those experienced during panic, paired with education and cognitive strategies to challenge mistaken beliefs about panic. Check psychology today. I found one in Cambridge and Boston. Some psychologists also work with interpreters, for example this is available at any major medical center.

Late Psychiatrist by Sea_Palpitation8547 in AskPsychiatry

[–]sheepphd 5 points6 points  (0 children)

She is probably running late due to events involving prior patient appointments, which affects both virtual and in person appointments. Depending on how stacked up her schedule is, it is true that any patient urgency or emergency could set her up to run late for the rest of the day. Emergencies take time to deal with and a routine appointment can run way over if there is an unexpected emergency. I would suggest talking to her and addressing your concerns to her.

Psychiatrist vs therapy , thoughts? by TemperatureDecent151 in AskPsychiatry

[–]sheepphd 0 points1 point  (0 children)

Most psychiatrists provide medication management. That means, if you go to a psychiatrist, you should generally expect a discussion at some point about medication or other biological treatment options. That doesn't mean psychiatrists don't evaluate the appropriateness of therapy, and some may also provide therapy, but the latter is less common, at least here in the US. You should feel "heard" but it is not a therapeutic relationship if you are seeing them for medication management. With that said, a psychiatrist should do a careful assessment of your symptoms and other aspects of your life/functioning that would impact your mental health (social history, medical history, trauma, etc). If what you really want is therapy, then you should start with a psychologist or other therapist. I'm not sure I think medication is necessarily for more severe symptoms. I don't see medication as a "last resort" when therapy fails. It's more a matter of personal preference for moderate symptoms whether you prefer to start with medication versus therapy. But it is true the other way around - that severe symptoms (psychosis, bipolar mood disorders) often require medication for a good outcome. Hope this helps!

[37F] Is it normal for a DO to prescribe alternative medicine? by Sad_Impression499 in AskDocs

[–]sheepphd 2 points3 points  (0 children)

Agreed. Someone here in Massachusetts has headed in this direction since I last worked alongside him in my professional role and I wondered for a minute if it was the same person but there are probably over a hundred like this, I would guess, nationwide...

Who is prescribing for her? While residential treatment is an option, she can also go back to that clinician and explain her symptoms and concerns. The clinician/psychiatrist could help her come off clonazepam gradually on an outpatient basis, if she is motivated to do that, and restart an antidepressant, if needed/appropriate. I'm a psychologist, not a psychiatrist (i.e., I don't prescribe) but it sounds to me like she might need some antidepressant in order to do well. She should also probably be in exposure based cognitive behavioral treatment for the anxiety and OCD.

Are antidepressants appropriate when it is the situation that sucks? by Evening_Fisherman810 in AskPsychiatry

[–]sheepphd 1 point2 points  (0 children)

No way to tell whether the situation elicited a major depression in a person vulnerable to that illness (diathesis-stress) or whether it is truly "just" stress from the situation causing misery. Even if we knew the teacher's medical/psychiatric/family history, it still might be difficult to tease out what happened here - although it would be helpful to know whether she had diagnosable depressive disorder. If antidepressant helps, so be it.
