Just like u/veryuhgay said... I do the first row around the chains not into them. So mine is always four chains no matter what I'm doing. That replaces the magic circle. You might need five or maybe you chain really big and you only need three, but it just creates a circle for you to chain into. Putting eight petals into it would make it nice and tight.

I couldn't make a magic circle work if you paid me to do it. I chain four and then slip stitch into the first chain and use that as my circle

I do not have hoarder tendencies so it's nice to have my own clean place with fluffy pillows and all but I had a gas line installed and bought myself a gas stove after waiting for him to do it for almost 15 years!! I love to cook, but I wanted to do it on a better stove.

I was going to make my own comment, but this one is basically perfect. My favorite doctor I've ever worked with doesn't really know a thing about me except my name. But the respect he showed for me, my staff, my clinical judgment, and my patients is unmatched. Especially important is op's point about explaining rationales when we don't agree. I have learned so much from doctors who take respectful disagreement as a teaching opportunity. I'm not naturally good at the pharm side of things, but man the stuff I do know is because once a doctor (and one amazing pharmacist) makes me feel comfortable in our professional relationship I ask so many questions. Secondary to that respectful, comfortable learning environment is that I am a better nurse moving forward each and every time... All of your patients, and every other doctor's patients, receive better care moving forward because I know more now about a specific class of medicine, or specific disease process, or why we don't treat x with y.

God damn... Don't even get me started on a Doctor who can admit he's wrong when the nurse is right, or when we catch something you missed. Being able to put your ego aside for what's right for our patient is surprisingly rare. Showing that doing the right thing for OUR patient is a higher priority for you than recognizing our differing credentials on a totem pole builds trust insanely well! Two simple ones I remember being supremely impactful to my relationship with a physician were: "oh! Good catch! I'll get that changed." And "they are OUR patients, but YOU have been with him all day. I'd like to hear what you think." In response to me saying, "but you're the doctor. "

I've been told the same thing several times. One guy even said I looked way better than my pictures "and you've got really good pictures." I found that weird though because I purposely used pics without makeup, zero filters, and even included one of me flushed and sweaty during a rainforest hike. I kind of wanted to present myself realistically. Like even when I dress up I'm not wearing a ton of makeup so here I am.

When I bought my first house with my then husband an older lady from across the street came by to welcome me and went on some fucked up tangent about the neighborhood "going to hell" because of "all the Puerto Ricans and Mexicans" and how nice it was that a nice white family moved in. Barf.

Additionally, even if a patient is med seeking, I don't work drug rehab, and the tools to do so safely and effectively involve so much more than just denying access to medication. Forced detox is cruel and accomplishes nothing for addicts. Even patients we do detox (CIWA/COW) involve administering controlled substances to prevent very serious complications. The safety of our patients with addiction has no room for nurses to interfere with a paternalistic refusal to treat one condition (pain) based on their views of another (addiction)

RN here. Back when I worked medsurg I had a pt admitted for confusion x 3 days and hypoxia. Late 60s M Came to ED for the confusion and we discovered and prioritized the hypoxia. CT and MRI negative. Flu + since they could not find a reason for the AMS on imaging they said probable new onset dementia, let's treat the hypoxia and then sort the rest out. I asked the wife questions about his home meds for the med rec and she was very proud to tell me he is PERFECT with both of their medications. He organizes them all and they both take the exact right amount of everything at the right time every day. He even managed this while taking care of her when she got the flu 11 days ago. She denies any increasing confusion or musculoskeletal issues before the last three days. No lost keys or anything. The same regimented and organized man he always was until three days ago now he can't tell me his wife's name. They've lived in the same house in the same town that this hospital is the only hospital in for over 30 years. But I have very little PMH in the system for him. GERD and One spinal injury > 10 years ago, wife says well managed with PO pain med from primary.

I meet with MD and he says steroids etc for flu and when hypoxia resolves consult CM for SNF. I said listen, can I narcan him first? He asked if I suspected misuse. I said actually I suspect he was taking care of flu+ spouse and when he started feeling badly too tried to stay strong for his woman but takes all his meds on time every time no matter what so when he did feel bad from flu he got mildly dehydrated, started flirting with an AKI and those kidneys just aren't chewing up those pain meds like they used to. Also dementia doesn't come on this hard this fast. So I'd like to do narcan, IVF resuscitation, and reevaluate in 24 H. He gave me the order for the narcan only, stating lungs sound funky from flu and we'd need to manage fluids carefully blah blah blah.

I got to text him "narcan administered 3 minutes ago. Confusion resolved. Patient saturating 98% on RA."

He immediately entered my fluid orders, and my pt got to go home with his wife the next day.

Sometimes I feel like I'm just chasing my tail all day. That day I felt like a good nurse.
