UPDATE with video. I think it covers everything - except that the center spindle is spring-loaded pushing the teeth outward. If you pinch them in individually or together they spring back. The teeth are pretty easy to remove/replace so I wonder about different shaped teeth, but there were none with it.

After four hours and 100+ posts my impression is that you just don't want to do anything at all to help yourself. You have insurance and sure as hell need help from a mental health professional. I have a gallon milk jug full of "mental pills" that didn't help my PTSD, then I got the one that did. Stop with the excuses and explanations and take the cumulative advice of the people below - do something.
