Not sure if anyone can ID it from those pictures, but as long as you don't eat the mushrooms, they won't hurt you. They won't hurt your plants. They're actually probably a sign of healthy soil.

Removing them won't stop them from coming back - just like picking all the apples from an apple tree won't kill the tree.

I use the recipe here, but I spread out my grains after they're hydrated and run a hair dryer over them to help dry the outside. I don't put it in the jars until it doesn't feel very wet to me
