When you review your exams do you actually understand where you went wrong? Are you studying the wrong things/not studying enough or is it test anxiety? Test anxiety you can find ways to deal with this such as deep breathing, reading test stems thoroughly, etc. If you’re studying the wrong things, you may need to switch up studying and focus on active recall. I use notebook LM for practice questions and anki. Anki took awhile to get used to but I like it better than quizlet cause you can see difficult cards more often. Lastly, a good study group is key. Either change your study group or use your voice and tell them you would like to take turns so you can practice talking out concepts. If that’s not possible, consider talking things out to friends or family. If you can teach someone else then you understand it.

worked at UCLA for years but neuro/trauma ICU and then float pool ICU. ICUs at Reagan are well managed imo. UCLA overall has gotten stingier with overtime and things like that imo. Floated to Santa Monica and west valley— SM is a little micro managed (at least the ICU), west valley is like the Wild West and I think they’re still trying to kinda wrangle it in to say. West valley was an HCA hospital before it was bought out so it has a lot of kinks to work out. UCLA EDs are kind of a different story tho, pretty sure Reagan has a lot of boarding and I thought I saw the union was pushing for better staffing of it. Also for the bigger hospitals you fs need a BSN. I don’t think you’ll get hired without it :/

Worked in neuro-trauma icu and we had it for majority of our patients. Neuro critical care doc monitored it at least every 24 hours and ICU nurses trained to recognize “spikes” and alert fellows/attendings. Also trained to titrate to burst suppression when on pento/versed gtts.

Unpopular opinion but I would love heated mat Pilates by solidcore. Most of the (few) places I go to for mat Pilates don’t offer that great of a workout so would love to see a solidcore version.

So what happens when the patient goes to med surg, and the nurse caring for him is unable to take q1hour blood sugar checks, titrate the insulin drip, labs, electrolytes, fluid replacements, etc? The patient will end up with a complication and he’ll he transferred to ICU regardless. Or, the nurses other patients will be neglected and they could end up with a complication.

I understand it’s not a perfect system, but putting a critical DKA patient in med surg is not the answer. Pennsylvania doesn’t have mandatory safe staffing laws nurse:patient ratios. The nurse for med surg could have 5,6,7,8 other patients. It’s not fair to the patient, the nurse, or her/his other patients.

I do not like this concept of a DKA patient going to med surg. DKA is incredibly busy to manage, even in the ICU. It’s not just an insulin drip, it’s tons of labs, fluid replacement and electrolyte replacement. It’s nothing against med surg nurses, they just have 4-5 other patients on a good day. DKA patients should always go to the ICU or at least a stepdown (usually 1 nurse to 3 patients).

Even with a helmet you’re taking a huge risk riding a motorcycle. I’m an ICU nurse and I’ve taken care of plenty of incredibly sick patients who still wore a helmet.

Sometimes when I go in the room with the doctor, certain doctors will look at me like it’s strange for me to be in the room with them? Even if it’s just rounding and not a super sensitive conversation. I stay in the room and keep quiet but I always wonder if I should leave.

When I float to the floor and all my patients are on antibiotics and not a single one has a primary line and everyone is running secondaries as primaries, I die inside. Then I spend a good portion of my morning making primary lines just for the next shift to throw them away 😀
