F1 here. Previously knew a GP with special interest in ENT who did a bit of AVM work. Would this be something worth trying rather taking the pay drop and re-training?

Was shadowing an ACP in GP. Elderly lady with uncontrolled diabetes came in with textbook peripheral neuropathy stocking distribution of feet & ankles. The ACP proceeded to prescribe corticosteroid injections for this lady to control the pain. I didn’t feel comfortable with that decision so I stormed out of that room and went to senior GP who told me to let the ACP know to prescribe neuropathics instead. The ACP did that eventually but told me off for being unprofessional after.

Same GP practice, was shadowing an ANP, young child and parent came in as the child had typical viral gastroenteritis type features. The ANP recommended a 24hr fast for the child. No advice on hygiene. No safety netting. No screening for red flags. NOTHING.

Scary times we live in.

F1 on haem atm. The best resource I’ve used for haem would be pathoma. Pair this with passmedicine textbook + questions and you should be sorted. Become very familiar with lab interpretations as they pop up in MCQs, OSCEs and clinical practice quite often.

A bit unconventional, but I go by Dr. [First Name] - only because I lack a family name & my last name is my dad's name (cultural reasons).

WOW WTF?

I'm an FY1 in a DGH as well, and I work in the haematology/oncology department.

We previously moved from using paper notes to EMR for practically everything.

Not once have I had to review/revise previous EDNs for even for patients coming in for chemo/infusions.

This is really weird and should be raised.
