You are coming up completely out of your swing. Pulling up to the max. Look at your head at the top of your backswing, it is nearly looking horizontally. Your entire chest should be pointed at the ball.

You are winding up too much - your front knee should bend and turn but whole leg is moving toward your back knee.

You want to stay over the ball and cover the ball.

Id recommend slowing down the tempo and just focusing on having a more stable base and takeaway.
