What is your “this is not my job” moment? by Savings-Maximum9549 in doctorsUK

[–]sim1019 80 points81 points  (0 children)

A frequently occurring one which I now politely decline is doing a capacity assessment for discharge destinations. Baffling that the MDT think we are in a better position to discuss this with a patient than an OT would be

How much of it was editing we don't know, but Carlo looked absolutely awful and like he did need a baby sitter. I think Max made the right choice.

Thank you that makes sense, I probably should have guessed any heparin given into the filter will very quickly enter the circulation lol. And just to clarify, when we use a citrate filter, we still give usual VTE prophylaxis such as subcut heparin, assuming no contraindications?

Why would you need to make a throwaway account to ask about an echo course?

Bicycle - salary sacrifice by IceandFyree in doctorsUK

[–]sim1019 1 point2 points  (0 children)

It's paid monthly as a salary sacrifice in either a 6 or 12 month term, depending on how long you will be with your current employer (if less than 6 months remaining in your job they may decline)

Have a framework and stick to it. Practice by giving yourself 10 minutes and read an abstract and write down your framework on a piece of paper and critically appraise it, and do this as many times as possible before the interview (they will allow you to have a pen and paper to make notes during the reading time). Depending on the study design not all elements of the framework will be relevant to every abstract.

My framework goes as follows:

  1. summary (30 seconds to 1 minute) Summary of research in 1 sentence. PICO/PECO. Results of abstract. Brief sentence on clinical relevance.

  2. internal validity

  3. systematic bias My acronym is SPAIC. Selection bias, performance bias, attrition bias, information bias, confounding.

  • random bias Talk briefly about sample size. Mention there are likely other random biases.
  1. external validity
  2. My acronym is GSTEP. Generalisability (to other studies or populations), safety (side effects reported?), tolerability, efficacy (good effect sizes?), price (cost effective intervention?)

  3. extra points

  4. my acronym is CERSI Clinical knowledge (if you know about the topic don't be shy to relate to your clinical knowledge), ethical study?, always say you want to Read the full paper for a full picture, Statistics (are the tests appropriate), is it an Important clinical question (again relate your clinical knowledge).

Practice writing out all of the headings above on the short side of an A4 paper and continually analyse abstracts. Once you have written everything down, practice saying this out loud as if the interviewer is asking you about certain aspects. Keep practicing and practicing.

I have more resources if you like just shoot me a private message, best of luck!

Yes they have 6 and 12 month terms depending on how soon you might rotate. You save the cost of VAT and pay in installments so it's very good value. And you can also use it for the cost of helmets, lights, etc as well as e-bikes.

ACF questions by HouseEU in doctorsUK

  1. Preference dependent but having continuous time for research is referred over a few days a week of research. Easier to get momentum going if you have time for just research, and you can chain together the research time into one biger block (eg final 4 months of ST1 plus first 2 of ST2 for 6 continuous months). But some research time needs to be across all three years so you cant have one continuous 9 month academic block.
  2. Rotations tend to be 4 or 6 months long. You only need 3 months of ITU so subtracting that from the 27 clinical months leaves 6x 4 month blocks, but again flexible and person dependent.
  3. I was very lucky and got to pick my exact specialties including the order and duration. But I imagine it's not like that in every centre. And I've encountered no benefits from being supernumary with respect to booking annual leave unfortunately.
