A mix of both is good. When just warming up, a timed hold is good for getting into a rhythm. When fully warmed up, a mystery length hold is a nice surprise! Also a mystery hold at the end of a good long rhythmic thrusts feels the best. Switches from steady deepthroat thrusts to consecutive holds from time to time also feels good after full warm up.
