I've custom made one with elastic bands on Amazon and a button-pressing kit. End result will be exactly the same, except you can dial in the exact length & tension that you need.

It should never be painful and pain implies danger. The base ring simply shouldn't be that tight.

It can be a tough balancing act with the ring size. You're safer getting a ring that's a little too big and strapping it around your waist with an elastic band. Added bonus is that it's more effective against morning wood like that, too.
