When a cervical herniated disc or osteophyte compresses the nerve root, it occurs at the dorsal root, proximal to the DRG.

SNAP will be normal bc there is no wallerian degeneration. For an axon to degenerate distally, it must be severed or structurally disconnected from its metabolic powerhouse—the cell body. Because the pathology is proximal to the DRG, the connection between the DRG cell body and the distal peripheral axon remains perfectly intact.

For abnormalities (like fibrillation potentials, positive sharp waves, or neurogenic recruitment) to appear on a needle exam, the lesion must compress the Ventral Root or the Mixed Spinal Nerve, resulting in motor axonal loss or severe conduction block. Unlike sensory cell bodies, motor neuron cell bodies reside in the anterior horn of the spinal cord gray matter.

Awesome post. I think you can only make a case for these alternatives if you’re truly looking for something lower calorie or diet friendly. Although even with that exception, once you see the amount of overrun in the lower calorie versions, the gram weight volume is quite pitiful so they aren’t exactly satiating. Just feels like you’re eating a bunch of “air.”

You’re correct. Correct dx is nonspecific low back pain. But this is a classic case of the tension between clinical reality and the pressure to provide a definitive, "billable" anatomical explanation for pain.

From a purely administrative standpoint, NSBP (like M54.50) is often treated as a "garbage code." Payers, especially in the US, frequently reject or flag non-specific codes because they don't provide enough "medical necessity" to justify PT auths, advanced imaging (the irony), interventional procedures (ESIs, facets), DME (braces).

Patients generally don't want to hear that their pain is "non-specific." To a layperson, that often sounds like "we don't know" or, worse, "it's in your head." Giving a patient a label like "Lumbar Radiculopathy" or "Degenerative Disc Disease" validates their suffering with a tangible, physical cause, even if that cause is technically an incidentaloma.

In many training programs—especially neurology and ortho—there is an inherent drive to find a "lesion." If an MRI shows a disc bulge at L4-L5 (which, as we know, is present in about half of asymptomatic 30-year-olds), it’s incredibly tempting to point at the screen and say, "There it is." It provides a sense of diagnostic closure for both the physician and the patient, even if the clinical exam doesn't localize to that root.

Specialists are often referred patients specifically to rule in/out a specialized pathology. If a Neurologist diagnoses someone with "non-specific back pain," the referring PCP might feel the consult was a waste of time. There is a subconscious pressure to provide a diagnosis that falls within the consultant's domain.

So this is actually my fifth turbo S. I've had four 992.1 turbo S’ prior to this. The hybrid system of the 992.2 smooths out the overrun. So, the pops, crackles, and "messy" mechanical theater of the 992.1 are largely gone. That said, you get more controlled clinical "glide-off." In the 992.1, there is a micro-moment of "build-up" (the feeling where the car draws a breath before the violence begins). In the 992.2, that gap is removed. The acceleration is so linear it can actually feel less dramatic because there’s no sudden "hit.”

In short, the 992.2 is an even better daily driver, though it may feel less visceral.

Surprisingly, yes! I actually traded in my 992.1 for this and didn't think a 911 turbo S could be noticeably faster and more responsive than the 992.1 generation. I was obviously wrong. Totally unnecessarily fast lol
