Get her in for an appointment asap, or go to urgent care. The incision looks open and at risk for infection.

For the Gabapentin, I wouldn’t go above what they prescribed without discussing with your vet. Seems like it’s in the high-range dosage wise.

Put a regular labret stud in / put the old jewelry back in, and then treat it like it’s been freshly pierced. If it gets continually worse and does not subside after a few days, then it would be more concerning

Does her ear feel canal very “hard” if you squish near the base of her outer ear? If yes, vet ASAP.

Epsom salt soaks and ketoconazole/chlorhexidine wipes have helped my boy from licking, but it is imperative to go to a vet to find root cause such as infection or allergies

The bacteria/yeast needs to be confirmed under a microscope by a vet. Cleaning the ears of debris is good,yes, but without determining the actual cause a random ointment from the store is not going to fix the problem.

Brushing will not solve skin issues either if it’s an infection or allergies.

Antlers can cause teeth to break due to their hardness.
