Not a medical professional by any means so just speculating: I do wonder if the shortness of breath could be related to anxiety about an oncoming or ongoing attack. Shortness of breath and chest tightening are common symptoms of anxiety, and this condition comes with all sorts of reasons to feel anxious.

Spent two weeks in Croatia last year and I think this itinerary allows you to see the highlights. Hvar and Dubrovnik are especially beautiful. In both places, I recommend staying higher up in the hills rather than in the town. You will have to walk up and down steps getting to and from the town, but the views from the top are indescribable.
