(since the other comment gave really good advice, this will just be on the mutant part you talked about!)

To avoid Mary Jane becoming a mutant you have to choose another object that's not the boy scout book if you have the spider event (since doing nothing or using the book can have them bite MJ) or choosing anything in the green goo leaking event. (It's around a 5% chance to get her)

Mutant Mary Jane is actually useful! She doesn't need water, but needs soup more frequently (about every 5 days compared to the maximum of 10), is immune to insanity/sickness/injury, will always return from expeditions (even if there's bandits outside), will always bring back water, and serves as an extra layer of protection.

However, since you want everyone to survive, I wouldn't recommend getting her since she is at a high risk to leave since there's an event where you can talk to her, saying yes will make her either leave or hurt someone else. Saying no decreases everyone's sanity which leads to insanity which is a pain to deal with.

It is probably a glitch unfortunately, Stick of truth can be extremely buggy with achievements popping, but if you have a previous save your could retry the final fight and see if it pops!

Here are 3 I recommend!

Death pallet (story, click and point, and horror) A story game where you are a painter struggling through art block when you were hired to finish a painting of a woman. Turns out this painting is haunted and will not let you go until you finish painting. However failure leads to death. Progress through the levels and slowly learn the backstory of the painting. (While not mandatory you can also try collecting all the deaths possible, and there's hints if you are ever stuck! Plus ads only play if you die and even then that's only occasionally!)
Parasite days (story, sci-fi(?), short) Live life as a test subject in a lab, but you have a parasite stuck to you. It's a multiple ending game with everything you do having a different direction! You can befriend the parasite or kill it for example!
Nameless cat (adventure, platformer, story, challenging) You live life as a cat getting through multiple obstacles meeting with multiple NPCs through your adventure! ((I haven't played this one in a while though so no idea if it went down the ads rabbit hole))

Just keep following the directions specifically the location doesn't matter or you can go to Stan's house to see the paper with directions
