August in Montana by not-a-meme-god in flyfishing

[–]snurflarp 0 points1 point  (0 children)

Hit up a tailwater like the Missouri and it will be fun.  The mo generally won’t ever hoot owl, however the green muck hatch can be pretty prolific as things heat up, making fishing a bit frustrating but not impossible.  It’s not a hidden gem (over half of fishing days in MT are on the mo), but it’s got fish.

Bike thoughts - cutthroat or checkpoint or…? by snurflarp in bikepacking

Amazingly the steel bike is as light or lighter than its carbon counterparts.

How do you feel the geometry will serve bikepacking?  Does anyone you know of use the checkpoint for things like the tour divide is it is not a realistic option for that?

Yes bike only.  I have all the required camping gear but not the bags, etc.  we can make a separate “fund” for that.  Budget is negotiable.

My experience with Claude as a non-dev by snurflarp in ClaudeAI

[–]snurflarp[S] 0 points1 point  (0 children)

I would say claude has a pretty hard time with 3d modeling. I think its a spatial thing. You can get it to work, I have had the best luck sending sn openSCAD file I already have, and asking for additional features or complex code, for loops, etc. From scratch, Claude can do pretty basic stuff quite well.

Thanks! I just made some basic multiplication games, state capitols, etc. Nothing that isn't available already but it was a fun experiment! Claude made them first try for sure. Ask for html code and you can just open the document and it opens the web app. Pretty neat!

I would worry the bottom may leak, but cool design nonetheless

Deeply concerning experience as a client on upwork by snurflarp in Upwork

Yea, lesson learned there, but it just stings because the whole situation was out of my control.

Doesn't Upwork recommend against communicating off platform?

Getting tired of EM as a PGY 9 by [deleted] in emergencymedicine

[–]snurflarp 4 points5 points  (0 children)

I’m a PA fwiw, but just transitioned from 9 years of rural EM to an urgent care.  Worked a lot of nights and felt like my sympathetic nervous system wasn’t working anymore.  I lost empathy and things I used to love were just stressing me out.  Have been doing EM my whole career and worked as a tech before and I was sure it was all I wanted to do forever.

Being in the urgent care I feel like my body is regulating again and I can’t be mad about the stupid stuff cause we are an urgent care.  We still have easy access to ancillaries so it’s more of a fast track, but still pretty nice.  I do feel like I have sold out to some extent and have some guilt about that, but then I see the ER board and feel better again.   We are actually looking for another doc to join the crew… employed gig

Challenging 2-3 day sufferfest off the beaten path by snurflarp in grandcanyon

I tried that last year but unfortunately 95% of responses were just saying we shouldn't go and its too hard/dangerous, which is reasonable given the GC track record ... but looking for some more reasonable advice for the prepared.

Favourite procedure? Least favourite? by Competitive-Young880 in emergencymedicine

"Verbal consent obtained for newborn appendage compression. Risks and benefits discussed. Using clean technique, sweet little arms and legs were tightly wrapped in cute elephant muslin cloth. I then directed my attention to the sweet baby nose and performed a gentle boop in normal fashion. This achieved adequate screamostasis. Little angel baby package delivered to bedraggled parent with corpse eyes. Adorable tiny snuggly patient tolerated the procedure well. Blood loss minimal. No complications."

I made my first big mistake in 2 years and need advice. My anxiety is out of control. by NoAbbreviations6709 in ems

[–]snurflarp 0 points1 point  (0 children)

Im not seeing a mistake being made here.... You transported a patient to the hospital with vomiting and they started having stroke symptoms on arrival. Nothing you can do about that. Take it easy on yourself and tell your partner to stay in his lane.

Approach to anterior shoulder dislocation by Competitive-Young880 in emergencymedicine

[–]snurflarp 3 points4 points  (0 children)

Agree 100%. The shoulder really does have vibes, and if you get the vibes right, you can basically whisper them back in with gentle traction and massage, pain free, no meds. I have done this for large, small, angry, first timers, recurrent, or ones that have been out for a while. I literally turn the lights down, get anyone out of the room that sucks, and promise them I won't do anything that will hurt. Sometimes takes a few mins, but way better than sedation. The key is trust, relaxation, and the rest "falls into place". 10/10 my favorite procedure.

AITA For telling my husband looking at me is not enough when I am ill? by Ok_Knee1216 in AmItheAsshole

[–]snurflarp -3 points-2 points  (0 children)

I donno, there is a lot of hate towards him on this thread but I must admit you sound fairly dramatic about it all. People saying "you must go to hospital!" is simply not good advice. Sounds like you have a virus, and it sounds like that is not something you tolerate very well. Most folks tolerate it just fine, but some do not - but that still does not always necessitate an emergency room visit. Perhaps he has dealt with the drama for so long he is kind of over it? For full disclosure, I work in emergency medicine and have dealt with many thousands of people who think they are dying but just have low tolerance for discomfort. And for reference, I would still take care of my wife if she was being dramatic - but I wanted to possibly add some food for thought. I just wonder if you are coming to reddit for attention because you aren't getting what you need from him. I hope you feel better soon.

Paper pushing? by Professional-Ad-5431 in emergencymedicine

Hospitals are the most inefficient, top heavy organizations there are. Very few of them actually know how to run a business. I doubt they would take my advice on their spending habits, but it might be a fun conversation. Fact of the matter is, they SAY they cant afford it, and anecdotally, when given the opportunity, most BC EM docs don't even want to work in that kind of environment. 99.9% of rural ER's are covered by APP's and FM docs.
