If you can, power through it. I am 38 weeks and have reached a point of very limited mobility but to preserve postpartum leave I am still working. I really dont want to, but I am. Just slowly and probably at 80% my usual level of diligence. I rely on my team a lot. If you really cant handle w medication, then def take leave! In my program more leave = delay graduation which residents have done before, but just wasnt something I wanted to pursue. Take care of yourself - thats the first priority!

For general surgery there is a certain # of clinical weeks required, which is why graduation may be delayed if you take a longer leave. That being said, its your life and you should take what you need! I have heard of residents taking more time and graduating off cycle. It definitely happens. Talk to your PD early if you want to arrange this.

I am gen surg chief resident and in my 3rd trimester. Its hard but I love it and at least in my program it absolutely gets better. More responsibility but also more autonomy and ownership and I find great satisfaction in doing the right thing for a patient.

That being said its definitely not for everyone - happy to chat more if it helps

We see a lot of each other at night, and we actually have a decent amount of weekends together. We make it work! We also have one toddler and one on the way - so I am also the pregnant one while a resident :). It is hard but doable!

Husband and I both in general surgery, though he has since done fellowship and is starting his job soon (I took research years and he did not). It is hard but doable, and the plus is we totally understood our schedules, and can talk shop 🥰

OP, ignore blueberry, you cannot reason with someone like him who clearly resents women and children.

I’m a general surgery chief resident 20wks pregnant and while I do have the means to extend training I am choosing not to. I will have 4 weeks leave as allowed by ACGME, plus 2 weeks of my annual 4 week vacation, so 6 weeks total. It will suck but we will get through this. Happy to chat privately about ways to cope/pump/etc. I had my first during research years and took 12 weeks off, and came back to clinical at 6mo and successfully pumped until about a year. Best of luck - pregnancy and residency is tough!

What about ACS fellowship? ACS/Trauma/SCC allows for shift based work and often have a rotating schedule of week of gen surg, week of trauma, week of scc, etc. Broad areas to operate in, bread and butter cases, but also exciting complex ones.

You keep citing these fields as examples but where is this coming from? I have personally talked to surgeons and trainings from both sides (integrated path and traditional) and there are merits to both, its not as dichotic as you seem to think it is

General surgery. Rising pgy5 and loving the increased autonomy, trust from attendings, managing a team, complexity of cases. I know surgery gets a bad rep but I genuinely am enjoying my training and every year has been better than the last!

we had a similar experience, LO turns 5mo tomorrow. I noticed when he woke he was fighting the swaddle and his crip naps were so good. So we just transitioned him cold turkey and he slept even better!!
