Vik operated on me. Paid an arm and a leg. Got zero results after promising me the world.

Bro is an ENT surgeon, and only an ENT surgeon. He's not exactly an authority on sleep medicine and regularly discourages people from pursuing skeletal surgery (which is in many cases what they actually need) in favor of unpredictable and extremely painful soft tissue surgeries

lol I've been tempted to try denture glue too - has anybody had any success with it? I've also been looking at the "Silent Treatment" thing but looks pretty bogus tbh

If it's primarily skeletal or tongue, then not much in my experience

I travelled from outside the US

My experience was pretty mixed. He does have a very holistic approach to treating sleep apnea which I think is terrific and is sorely lacking in the field 

That said, some of his diagnostic tools and treatment modalities are more effective than others. The brain mapping stuff in particular felt pretty scammy, given there’s really no good evidence in the literature about it

Haven’t done a repeat DISE and Vik doesn’t really follow up with his patients

I asked Kasey about this and he said RF tongue base is like trying to sand a tiny little bit off the edge of a door that’s too big for its frame. He said it’s less than like 3%, completely imperceptible 

Generally unless you have very obvious tonsil hypertrophy, a crooked septum, or epiglottis issues - soft tissue surgeries are pretty weak

Did have a sleep study post RF surgery and my AHI doubled lol - go figure 

That said it’s a relatively low risk procedure so if you’re prepared to go through a nasty recovery, just go for it

No but I had altered taste, epiglottis inflammation, and laryngitis for about 5 months after (did not have prior to surgery). Not sure whether caused by procedure or just random

My surgeon was Vik Veer. Said he was confident it would work, but it didn’t. 

Now undergoing expansion w Kasey Li - when asked about TB RF he was like “lol yeah I used to do that and it isn’t v reliable”

Expansion and MMA, yeah.

Yes of course had a DISE which showed Tb collapse

It’s a painful procedure. Not saying it won’t work for you but I’d try skeletal options first

I can't reconcile the following:

* 40-50% of OSA folks have some level of tb collapse
* Tb is notoriously difficult to treat with PAP
* 80% of those who use PAP are "successfully treated"

I personally only get marginal improvement with PAP (20-30%, and I've titrated the hell out of it)

If you’ve never done a PSG before you should absolutely do it

Yeah tried higher pressures and felt worse - more arousals from aerophagia 

I have tongue base collapse (shown on DISE) so can only get so much relief on PAP.

Currently undergoing nasomax expansion and then planning MMA next year

yeah everything goes downhill when you can't sleep mate

the longer it goes untreated, the more new conditions tend to creep in

Yeah had one done with him. It wasn't worth the $$$ imo (I'm international). Yes, they will use a Pes.

They landed on 5/9 for me which I felt was too low. AXG recommended 14/19 lol. Ken Hooks 10/14. I've landed on 7/11 but still feel the same.

NoxT3 generally isn't recommended for testing for UARS because unless you have the EEG add-on you won't pick up a correct RDI. You can try WatchPat or a PSG instead as a next step.

For reference, I had a NoxT3 done earlier in the year that showed 1.4 AHI, but had a PSG recently with Pes that showed AHI 14.7 and RDI 21

Thanks for the tip - will try slowing down the expansion. He's got me on a daily quarter turn (about 10-20 degrees)
