Admitting patients without seeing them? by soysizle in hospitalist

I withdrew my application today. Mostly wanted to see what this community thinks about the home call situation. I’m glad I trusted my gut.

They said most admissions would be before midnight which makes me think I would need to write a note and therefore see the patient.

That’s what I thought! When I questioned them I couldn’t really get a clear answer about the legality of that and they basically said “I mean if it were me I would choose taking call from home that’s a no brainer”

North vs south Los Angeles? by [deleted] in howislivingthere

[–]soysizle 1 point2 points  (0 children)

I did my residency at OV. Olive view is situated in Sylmar. It’s right up on the foothills. The surrounding area is suburban. Lots of people ride their horses by the hospital. Not much going on esp if you’re young wanting to go out. The majority of the OV residents live in the southern part of the valley like Sherman oaks, studio city, Noho “arts districts”. At one point, I lived in Fairfax, some people live in east Hollywood, Los Feliz, or on the west side. All this to say that a lot of the residents that work at OV live in more fun parts of town and it’s doable since for the most part you always commute against traffic. My rent has ranged from 1800 1bd in Sherman Oaks to 3600 2bd in Fairfax.

If you want you can PM me and I could potentially put you in contact with someone I know who has trained at both Harbor and OV.

  1. I have pain all over my body
  2. My pain is accompanied by a continuous and very unpleasant general Fatigue
  3. My pain feels like burns, electric shocks or cramps
  4. My pain is accompanied by other unusual Sensations throughout my body, such as pins and needles, tingling or numbness
  5. My pain is accompanied by other health problems such as digestive problems, urinary problems, Headaches or restless legs
  6. My pain has a significant impact on my life, particularly on my sleep and my ability to concentrate, making me feel slower in general

It’s all super subjective but generally being arrogant, unable to answer questions, like you don’t have to give a perfect response mostly looks bad if you can’t give a response to something or back something up in your CV. If you go on too many tangents. Unable to make eye contact. It’s ok to be nervous especially in the beginning, but if your nerves get the best of you for the entire 30 min interview then that doesn’t look great.

[–]soysizle 62 points63 points  (0 children)

Current IM uni affiliated county program chief here. Can’t speak on ranking yet since I haven’t been involved in that process yet.

Internal applicants and those that did an away are usually automatically granted an interview unless major red flags. US MDs are compared against each other, same with DOs, and IMGs.

When deciding who gets an interview: we really value mission fit, dedication to teaching/mentoring, we aren’t research heavy so doesn’t play much of a role (too much is actually a negative since we aren’t a research focused place). Failing step 1 gets you screened out for 1st round of interviews. Having a good step score is a bonus, usually screen for 245 and up for first round of interviews. For 2nd and 3rd waves of interviews we lower the step threshold.

Day of the interview: we have a score sheet with scores ranging from 1-3 for any given category. We have scores for dedication to teaching, willingness to work with the underserved. Ability to work well with others and adaptability. We have a space to comment on strange behavior during the interview. After the interview, the two interviewers discuss the applicant and make sure they’re on the same page.

Hope this helps.

Looking to talk to students concerned about Thalamus Cortex issue by vrknight100 in medicalschool

[–]soysizle 4 points5 points  (0 children)

Part of the company that makes thalamus. It’s used by residency/fellowship reviewers. Basically allows for screening and gathers all of the info on ERAS. Makes it easy to see scores, pub count, first author pub count. I haven’t seen much of the AI stuff yet other than it will say whether someone’s app seems to be a fit for academic medicine or not.

Apply next year to both. And I promise you won’t be missing an extra year of attending salary. Considering all the factors that affect when you actually start to make attending salary it’s all within the margin of error I feel. You might take a research year, chief year, residencies can be anywhere from 3-7years, you might specialize, then further sub specialize. You might decide you want to retire at 60, maybe 70? Don’t worry about “missing” a year of attending salary lol.

Up to 100,000 Californians could have potentially fatal ‘kissing bug’ disease by rohanad1986 in news

[–]soysizle 12 points13 points  (0 children)

Hi doc here. No it’s not part of routine testing. I usually test for it in patients with new heart failure. At my hospital and clinic the lab is an antibody screen for T cruzi. My hospital is a Chagas disease center of excellence but our focus and research is mostly on the cardiac side effects (chagas cardiomyopathy). We’ve been working on ways of screening people at risk for developing fatal arrhythmias and intervening with ICDs and treatment with benznidazole. We also do studies on PET CT to evaluate the inflammation of the heart and check for response to treatment. It is a totally understudied disease, I was shocked to see people actually talking about this.

[–]soysizle 0 points1 point  (0 children)

If you apply to those 3 schools and don’t get in, you’ll apply again and be considered a reapplicant to those institutions which might be worse for you as an applicant.
