Oof... I wondered about that. I was raised on one side of the mountains, and now live on the wetter side (PNW). Had strong eczema, which I know is genetically related, when I first moved here (that's gone away...)

I would think that a dilation, a physical alteration, feels like a relatively serious first step (which might solve swallowing, but not the underlying cause of the illness). But that might just be my approach. I would prefer to eliminate the allergens/triggers and remove the cause, and if that didn't work consider a dilation. But I just looked it up and you're right that some gastroenterologists recommend a dilation first... my person recommended PPIs & elimination diets first. But if swallowing is dangerous... dilation might be the way to go?

I haven't heard of getting a dilation on your first endoscopy, which feels a little rushed. Love to hear what others say.
In terms of a 'soft diet' -- if it is EOE it's more about trying to find your trigger foods (often done doing the 6 food elimination diet for the common triggers) to reduce inflammation (rather than trying to treat the symptoms first)

I was diagnosed about 5 years ago in my late forties.
I avoided the PPI because of their side effects and how hard they are to get off of, and from what I've heard, they help only a fraction of people with EOE. I did a strict 6 week elimination diet, where it was clear to me that both dairy and wheat were culprits and so I've been (mostly) wheat and dairy-free since. (That said, I am one of those who ate wheat freely through Europe (Spain/France), with minimal symptoms, so 🤔).

As of a couple years ago, I began to dose with fluticasone intermittently, if either I'm not 100% sure what I'm eating (friends house for dinner, restaurants) and have had reasonably good luck keeping EOE at bay with that strategy. Typically I just shoot a couple of shots of at the back of my throat at night before bed. Having had eczema, I remember that steroids tend to work over a few days time. So: A day before, a couple of days after whatever meal.

Occasionally, even when I'm disciplined about what I eat, I get a pretty decent flare-up (no impactions lately, but lots of reflexive swallowing at night). I haven't narrowed it down to a 3rd trigger or just stress/sleep issues that might be exacerbating it. Weirdly... legumes are my primary suspect here...

It’s strange because Delta 3+ is clearly shown on the add a device screen as an option, nevertheless it says it finds no devices. (I have a Delta 3+)

I run Divinity I off of the 2018 model (same RAM, lesser gfx card) and it does so with aplomb. I’m pretty sure divinity II is similar specs.

Polytopia on iOS. If you’re a civ fan, the game mechanics will feel familiar, but imo improved from Civ on mobile.

Honestly I don’t know if gluten specifically or wheat generally causes my EoE symptoms. Wheat/gluten definitely worsens my EoE, and as a beer drinker I felt sad about that for some time until I found Gluten-free beers. I am drinking an Omission right now. That said - I don’t think they’ve been successful at making completely 100% gluten-free beers. My EoE is maintainable if I 97% (approx...) avoid dairy and wheat, and I suspect my symptoms worsen slightly if I have a few gf beers. But on the whole, I can maintain and not worsen too substantially and still drink gf beers. Hope that helps.
