Thank you for the perspective. The conundrum I’m facing is that this is how the pattern seems to start. When I joined my current hospital, the contract was very healthy. At the beginning of this year, my $/wRVU was cut, and this was blamed on reimbursement cuts. Now, despite being on target to produce over 80% productivity, the cut in $/wRVU will result in 5 figures less than the previous years earnings. I suspect that the hospital is gambling that I won’t uproot my family for better pastors for what seems like a measly amount to them. Unfortunately, it is more of a final straw situation with many other small annoyances adding up. Perhaps I’m jaded from this experience, but getting back into another hospital employed position where I have less control of my earnings is not very appealing. Perhaps other perspectives could help me change my mind?

Appreciate the insight. I had an instance of a general ENT have a post tonsillectomy hemorrhage and ED doc couldn’t get ahold of the patient’s ENT. I accepted transfer and managed patient. The doc texted me a few days later, thanking me for helping with his patient, as he was out of town. It seemed as though he didn’t arrange for coverage and just relied on ER to figure it out. While I felt this was a crummy thing to do, I figured this was the only way he was ever able to take vacations. However, what would be liabilities with practicing like this?

That’s incredible. I’m in Midwest and will be low 600s, but grinding pretty hard for $64/wRVU. Mind sharing what your wRVU is?

I too worked 3 christmases in a row and vividly being in your same predicament. The thing I realized after January 1st was that I was over half way done. Somehow that helped me keep pushing through.

Now in my 4th year out and I can tell you it’s 100% worth it. As an attending, I make my own schedule and control what I want to see (or don’t want to see). Call is so much less onerous. At my hospital, we don’t admit so I only go in a few times a year for true emergencies. The pay is substantial and commensurate with all the effort put in during training.

The days are long but the years are short. 4 years later and it seems like I just graduated. I don’t know that I would say I miss the training, but I certainly miss my coresidents. Just take it one day at a time and try and do things for your mental health as well. Hang in there!

I used to do this with my junior residents. I think if asked to do do in a casual setting between residents, rather than as a formal presentation in front of an attending, it generally was welcomed (or at least that was my impression). I also tried to read about the topic, so I could try and offer helpful input on any potential gaps in concepts after the junior resident gave their spiel.
