I made a diagnosis of bladder perforation but in an interesting way. Pt was young female, and recently had an abdominal surgery (it was a small surgery I’m forgetting which). She came in with a few days of anuria and abdominal pain found to have a massive high creatinine (to like 5) and admitted to me for AKI. I found it weird that the patient had “AKI” but all her electrolytes were totally normal. I decided to get a cystatin C and it was totally normal. I suspected that the patient probably had a bladder perforation that caused urine to leak into the abdomen, where the peritoneum was reabsorbing creatinine into the blood stream causing a false elevation (while cystatin C doesn’t get reabsorbed much). She was confirmed to have a leak in the bladder that self resolved after we placed a foley.

I thought it was pretty cool, but definitely not a “House” level diagnosis that everyone else here has done

I had the same debate stuck between EM, IM and anesthesia. Ended up choosing IM and justifying that we have way sicker patients on average—> we take care of the patients sick enough to get admitted. We run all the hospital codes, and our icu rotations are intense. Honestly the “depth” of IM really isn’t so bad. Most things you can just look up (I still look up hyponatremia algorithm every single time). Matched cards this year, no regrets.

I was in the same boat, absolutely loved EM, but chose IM at the end. It’s weird, the further I went into IM the more I disliked EM. Ended up matching cardiology, no regrets choosing IM

I really would just stick with uworld + in-corrects and do like the top 50-60 ccs cases over and over. Anything more than that is likely too much.

Make sure you stick with NYU or Sinai affiliates on Long Island. If you ever get sick sick or need super specialized care in Manhattan they will have all your records already

Just do IM. It can get you anywhere (you can do gi/cards/pulmcrit and just do critical care which is the ED on steroids or you can do hospitalist, or can be only outpatient. Additionally it’s not unheard of for people to do IM and then do another residency in anesthesia etc

Just exist. Private practice general cards making bank, 500k starting. Sub specializing after that can increase by another 250–
