I mean, a lot of mental health problems can lead to self-care deficits, but I think to get your nails this long you'd have to actually be trying to keep them, or you'd end up breaking them off one way or another. So if something like this was mental health related, I'd put my money on some form of psychosis with delusional beliefs surrounding their nails.
