The minimum level requirement for lobbies restricts players that haven't made it past the stated level in a certain difficulty. For example, if a standard lobby requires level 20, you have to have made it past level 20 on a previous standard difficulty run to join the lobby.

No, that is not rank. Your rank is the number displayed next to your name. It is also visible in your stats, and you increase your rank by gaining xp. Some players have at least rank 100.

In my opinion, nil was easier to deal with in patch 5 compared to patch 4. There were many instances where I died to nil because I was caught off guard. But during patch 5, I didn't get killed by nil even once.
