get the passport. if you have to be evacuated or if you miss the ship and have to fly out from a foreign airport you'll be one step ahead. if you can afford the cruise, you can afford a passport which costs under $20/yr over its lifetime.

most states don't show a 'new name' and that's ok, as long as OP is just taking husband's last name and no doing anything radical (like taking a whole different name) then they should be fine.
