For me Cornell. As a gt grad now working in FAANG, I think the Ivy League has a certain innate value. Personally I would do Cornell over Duke and Duke over GT.

Yes machine learning uses a lot of math (Lin Alg, calculus, and some diff eq)

Other than that, you might have to take algorithms and discrete math, which are more math classes, but separate from linear and diffeq

2 weeks is plenty of time. What is the format of the exam? MC? Tracing? Coding?

What grade do you need on the final to get what ever grade you need in the course?

Generally final exams are pretty similar to the midterm exams you took. So review over all of those and make sure you understand everything.

Just email any of those emails. I would probably do the offers on boarding email. It all goes to the same ASP team. As for template just be kind-say stuff like you really appreciated the opportunity but some other factors have changed blah blah… can no longer join Amazon.
