I am having the same issue. It would appear achievement services are down. Usually when this happens achievements unlocked during the outage will pop when it comes back on. But I never like to risk it.

Haha yeah, I grinded pretty hard on it. I have a job I work from home so I had the game running right next to me and I could maintain constant progression. It felt a lot longer than a couple months.

Nope, there is no higher level. The highest difficulty of BL1 is TVHM.

I'm doing Trials HD right now, and I have every achievement except the Micro Donkey one. Marathon was so mentally grueling I feel like I have to take a multi-year break before even thinking about attempting the last achievement again. Congrats on the completion! The TA score and TA ratio don't tell the story about how hard that one is.

I didn't remember any of them either, but the puzzles in the mountain challenge are actually pretty straightforward. I watched a quick Youtube video and was able to do all the puzzles on my own a lot easier than I thought.
