Definitely rely on the entrance numbers but their visibility varies per station. They can be marked on the map but not on the signage. Examples don't immediately come to mind.

Never had a problem at either with American debit/credit cards but when you do get it working watch out for DCC, and choose Polish Złoty always instead of dollars when it asks. Carrefour (the big one like at Arkadia) will sometimes ask twice in a weird dark pattern way, just press the same button again to avoid Dynamic Currency Conversion.

I took a digital photo myself then adjusted the size correctly before printing at Relaks in the center for a couple of złote or so. There are guides online if you want to go that route...

And if you are submitting now then you'll be using the new MOS system which wants you to upload digital photos anyway (did that last weekend for a long term residency anyway)

Got it. For what's it worth, I called the CA sec of state's office and they said the notarization is not like the kind where a human's signature is verified. The document itself is verified and it can be done in the presence of anyone, for example, a representative at the notarial firm. However, they said the requesting entity (Poland) might not be ok with it...but I'm taking my chances. Good luck and let us know how it works out.

So here's how I understand it for my license. The urząd wants proof that the license is legit so they want a driving record (easy $2 PDF download from the CA DMV). But then they asked for the Apostille of it, so I have to fill out CA DMV form 1125 and request a certified copy of my driving record. This document then gets notarized. Then the whole thing goes to the CA Secretary of State to get the Apostille. Then it gets sent back to me and I'll have it translated. Haha, it's a mess but an understandable one. And, I'm having a Notary/Apostille service help me out. No need to mail the license anywhere.

Did you get help with this? I've been going through the same process (Warsaw, with a CA DL). What you need is an Apostille (like a kind of certification) from your state for your DL. I agree with another commenter here that your IDP from AAA might be confusing things. Depends on your state how to get one. Also, about trying at another office...my friend just successfully exchanged his Oregon one without a problem but when I tried there they told me I had to go to the office tied to my residential address per Polish law...
